Consultation Fee
Our consultation fee is $50 for each procedure. This fee is processed at the time you schedule your appointment and will be applied to the cost of your procedure should you decide to use our services. We are more than happy to provide a solution for your needs. We look forward to addressing any questions and concerns you have about permanent cosmetics!
Cancellation/Rescheduling Policy
We follow the strictest sterile guidelines set forth by the CDC in setting up our permanent cosmetics procedures. Each permanent cosmetics procedure set up is specific to each client’s needs and involves time in technician planning, stocking of sterile supplies, and setting up specific to your procedure. Therefore, we require 48 hours notice to cancel or reschedule your appointment to avoid being charged a “no show/broken appointment” fee of $50.
We need a valid credit card to confirm and hold your procedure appointment. We process a $50 non-refundable deposit on your card at the time you schedule your appointment. By scheduling your appointment, you agree to our cancellation/rescheduling policy and authorize Shelly Bohman, Permanent Cosmetics of Utah to charge your credit card in the event you do not uphold our cancellation/rescheduling policy. Unless your appointment is broken or you do not show up, this non-refundable deposit will be applied to your procedure cost.
Remember when you do not cancel or reschedule your appointment in a timely manner, it affects three parties.

3) and other clients.
